Job Description

Chestnut Park at Cleveland Circle is looking for caring and compassionate LPN's or RN's to provide exceptional care for our residents!

We have Part Time and Per Diem opportunities available. Hours to include alternate weekends and holidays. Additional hours may be available across locations!

Responsibilities:

  • Coordinates care needs with community providers via an effective case management process
  • Measures and accurately records vital signs where appropriate as per state regulations
  • Accurately assesses, communicates, and documents residents’ status
  • Successfully implements the plan of care
  • Administer resident’s medication according to physician’s orders

Job Requirements

Requirements:

• Graduate of an approved LPN / Licensed Practical Nurse program or RN / Registered Nurse program (per state requirements)
• Current state licensed as an LPN or RN (as required by state regulation) and CPR certification
• Previous experience working with the elderly in a paid or volunteer position highly desirable
• 1-2 years nursing experience required or other applicable experience considered
